We use standardized assessment tools whenever possible to identify home hazards, fall risk, performance of activities of daily living, adaptive equipment and accessibility needs, caregiver support needs, and more. If further physical evaluation is appropriate, we will assess for range of motion, strength, muscle tone, functional visual impairment, and can offer a basic cognitive screening. We will use this information to identify the most appropriate adaptive equipment or home modifications to enhance your or your loved one’s independence and quality of life.
The assessment takes several hours to complete. We accommodate your schedule and the assessment can be done at a time of day that works best for you. We find our clients enjoy the assessment process and find it positive and empowering.
**Due to the COVID-19 pandemic, we will do all we can to minimize exposure. We may be able to complete a portion of the assessment online via Zoom or Facetime. In some cases, it may be helpful if a family member or caregiver is able to assist. In-home assessments will be completed following safety protocols for social distancing, mask wearing, handwashing/hand sanitization or glove use as appropriate.
PROVIDE WRITTEN RECOMMENDATIONS
We will provide you with the assessment results and recommendations. Personalized recommendations for safety equipment, home modifications, and adaptive techniques will be included in the evaluation. Our recommendations will be based on your personal concerns, ergonomics, and home environment; not ADA standards which are generally established requirements for public use in commercial spaces and may not be adequate to meet your personal needs.
FACILITATE IMPLEMENTATION OF HOME MODIFICATION RECOMMENDATION
We can take it to the next step and manage the project through to completion, either working with your contractor or handyman, or we can help you identify a licensed and insured contractor. This would include determining scope of work and obtaining bids from several contractors to help you identify the right person to complete the job. We can help you get the job done, from very basic grab bar installations, to installations of ramps, lifts, or remodels. We are also able to work with established vendors and suppliers locally and out of the area throughout the installation process.
